Abstract
Background: Although hemodialysis is one of the most useful methods for control uremie and saving the life of more than 100000 patients , But it has been found this treatment method will not able to solve all patient's problem .Counseling and education can encourage patients to participtate in self care design , increase their self steem. The purpose of this study was to investigate the effect of counseling and education on biopsycho- social problems of hemodialysis patients referring to hemedialysis counseling clinic. Material and Methods: In this semi-experimental study data collection was performed by using questionnaire,checklist,close observation, interview and physical examination on 92 patients who were undergoing hemodialysis, prior and after the intervention. convenience sampling was selected according to intervention . Findings: The result has shown that most of the samples are men 51/1 % with the age mean of 47 and duration of therapy for 59.8% of the patients was 9 years, twice a week .The cause of ESRD was hypertension in 46.7% of the patients. A significant difference has shown by willcoxon test in mean average of physical problems of patients before and after the intervention specially on circulatory system ,but no difference determined in shunt or fistula site which was prone to infection . The most social problems before the intervention were job loss 65.2% , which was changed to 55.4% after intervention. From the point of psychological problem in 51% of the patients before the intervention depression was the most , which decreased to 36.9% after the intervention. Lab results has shown some changes in level of uric acid , creatinin, sodium , potassium and albumin which a significant differnce was determind by pair comparison test (P=0.0001).In the level of calcium phosphorous and urea no change has been shown before and after intervention. Systolic blood pressure was changed before and afterr counseling (P=0.0001). Conclusion: This study has shown that most of the patients were suffering form physical and psychological problems and counseling can reduce these problems and improve the patient's quality of life.
